Single biggest structural relaxation in the corpus; high-yield appeal lever for denials issued against older language.
Esketamine tightened in 2025 (CPB 0950). 02/27/2025 review introduces "within the past 5 years" qualifier on the 8-week trial requirement — meaningful recency restriction on prior failures.
